It is now possible to make, cancel and check appointments on line.  It is also possible to request repeat medication on line (as well as via the website).  Please come to the surgery to register for this service.

Maylands Patient Group 

In January 2011 the surgery set up a patient group.  The aim of the group is to provide a forum for our patients to discuss health promotion and local voluntary and social activities.  For more information, please see Items on the right of this screen, or contact Ruth Thacker, Assistant Manager.

Vaccinations

The surgery now offers the Yellow Fever vaccination

New Patient Information

All new patients are given a health check with a nurse or a healthcare assistant. This is an opportunity to obtain important medical history until your notes arrive from your previous GP. The health check must be booked within one month of your application. Everyone aged 5 and over should have the new patient health check. Children under 5 should be seen by a health visitor at the Elm Park clinic (telephone 01708 796066).

If you change address to one outside of this boundary you may be asked to change practice.
