It is now possible to make, cancel and check appointments on line.  It is also possible to request repeat medication on line (as well as via the website).  Please come to the surgery to register for this service.

Patients, we welcome your input to help us develop surveys, services, local  priorities and take action.

Maylands Patient Group 

In January 2011 the surgery set up a patient group.  The aim of the group is to provide a forum for our patients to discuss health promotion and local voluntary and social activities.  For more information, please see Items on the right of this screen, or contact Ruth Thacker, Assistant Manager.

New Patient Registration

NEW PATIENTS WILL BE ABLE TO REGISTER AT MAYLANDS HEALTHCARE FROM THE 1ST APRIL 2017

register_nowThe doctors welcome new patients who live within our practice area.

New patients will be asked to complete a New Patient Questionnaire.  This is because it can take a considerable time for us to receive your medical records.  You will also be asked to provide your medical card (form GMS1 to be completed if this is not available) photographic ID, ie passport or driving licence, current proof of address (such as utility bill).  All overseas patients wishing to register will have to provide visa status also, even if they have previously had a GP in the UK. 

When complete, please bring all documents to the surgery where they will be checked and identification documents copied.  You will then be advised by letter when to book for a new patient check with the nurse.
